Is Greenleaf an In-Network or Out-of-Network Provider?

Greenleaf Soft Tissue Therapy is considered an Out-of-Network provider. 

How is my insurance billed?

Greenleaf Soft Tissue Therapy does not bill your insurance directly. If there is out-of-network coverage available for you, we will supply you with the documentation that you would need for you to submit to your insurance company in order to seek reimbursement. 

Greenleaf Soft Tissue Therapy is not responsible for the determined amount of reimbursement you receive from your insurance company. 

How much will I owe at the time of service?

60 minute - $115

30 minute - $60

The client is responsible for paying at the time of service. 

What do I need to do to schedule my first Medical Massage?

You would first and foremost need to verify your insurance coverage to see if your plan has out-of-network benefits available.

Call the Member Services phone number on the back of your insurance card. When you call speak with a representative, be sure to write down their name, then proceed to ask the following 4 questions:

1. "Do you cover 97140 - Manual Therapy if performed by a Licensed Massage Therapist?" (If their answer is no, they will not reimburse you for our services.)
2. "What are my Out-of-Network coverage rates?"
3. "Can you please send me an e-mail verifying the Out-of-Network coverage for this service?"
4. "How do I submit documentation to my insurance company to seek reimbursement?"

If your insurance coverage is sufficient and you would like to schedule your first Medical Massage appointment, please contact us by phone.

You would then need to get the Prescription / Letter of Referral form signed by your physician to bring with you to your first Medical Massage appointment.
